The Chronic Disease Management programme aims to prevent and manage certain long term diseases. It is available for patients with a medical card or GP visit card and have a diagnosis of one or more of the relevant medical conditions: Asthma, COPD, Type 2 Diabetes, Cardiovascular Disease including Heart Failure, Angina, previous heart attack or stroke and Atrial Fibrillation.
We deliver this programme with twice yearly checkups and blood tests. The programme is designed so you and the GP can monitor your condition, identify early treatment and support you to manage the condition. It is an opt-in or opt-out service, and we would encourage all our patients to engage with our service to increase their overall health.
